Endodontic Canal Negotiation

Canal negotiation establishes the initial pathway from the access cavity to the apex before any shaping begins. The negotiation pathway determines the final canal shape — errors at this stage cannot be fully corrected by subsequent instrumentation.

Why Negotiation Precedes Everything

Canal negotiation is the first mechanical step after access preparation. It establishes the initial pathway that all subsequent instruments will follow. Attempting rotary shaping in an unnegotiated canal significantly increases the risk of ledging, transportation, and instrument separation.

Hand File Sequence

Small hand files (size 6, 8, 10) are used to negotiate the canal to working length before any rotary instrumentation. Sequential advancement through file sizes confirms patency and begins to establish the initial pathway without forcing instruments against the canal wall.

  • Precurving small hand files improves negotiation in curved or calcified canals.
  • Files should be advanced with a watch-winding motion, not apical pressure.
  • Resistance at any point should prompt reassessment, not force.

Patency Filing

Patency filing — passing a small file (size 10) 0.5–1mm beyond the working length — maintains apical patency and prevents apical blockage from dentinal debris accumulating during preparation.

Calcified Canals

Calcified canals require patience, copious irrigation, and sequential file advancement. MD-ChelCream significantly improves negotiation by chelating the canal wall and reducing instrument friction. Forcing instruments in calcified canals is the primary cause of ledging and false canal creation.

The Cost of Negotiation Errors

Ledges, false canals, and early transportation created during negotiation cannot be fully corrected by subsequent shaping. They compromise irrigant delivery, obturation fit, and apical seal quality.
